Dental, medical, and other practitioners, or their practice staff, can refer to MOSS by submitting an online referral form: Make A Referral

• Reason for referral
• The patient’s contact information including mobile number and email address
• If available, a copy of their most recent large jaw X-ray (OPG). Please include the date in which the large jaw X-ray (OPG) was taken

MOSS will then contact the patient to complete the online MOSS Patient Registration Form in order to be eligible for a consultation appointment.

MOSS also accepts referrals from patients (self-referrals) by completing the MOSS Patient Registration Form found here or scan the QR code below.
